| Medical Social Worker, Case Management POSITION #066 |
Provide patients, families and vulnerable populations with the psychosocial support needed to cope with acute, chronic or terminal illnesses. Services include advising family care givers, providing patient education and counseling, discharge planning and referrals for appropriate community services or out of home placement. The position reports to the Case Management Director. Masters of Social Work (MSW) required. Current Masters of Social Work license in the State of Idaho will be necessary for incumbent.
Must be competent in interpersonal relations. Must possess the ability to be flexible and prioritize crisis situation, work tasks, etc. Must have ability to work independently with minimal supervision. Must have thorough knowledge of community resources and crisis theory.
Minimum of 1 year work experience that includes counseling and crisis management. Experience in a medical health care setting desired. Experience in acute hospital emergency department preferred.
