Medical Records Request

Gritman Medical Center Health Information Management (HIM) Department is responsible for keeping complete and accurate information for each patient. To obtain copies of your health information (medical record), please see below or contact the HIM Department at (208) 883-6092.

You may also access your electronic medical record by signing up or logging into Gritman's Electronic Healthcare Portal powered by FollowMyHealth.

To request copies of medical records from Gritman Medical Center, including records from our Family Care Clinics, download and complete the Authorization to Release Protected Health Information. Failure to provide all requested information, including a photo copy of state drivers license or state-issued identification, may cause a delay in processing your request.

Return the signed authorization form to Gritman Medical Center by one of the following methods:

In-person: Enter Gritman Medical Center through the Patient Billing entrance (located on the north end of the hospital near the corner of South Main St. and East 7th St.). Take the White Pine elevator to the fourth floor. On the fourth floor, exit the elevator to the right. HIM representatives are available 8:30 a.m. – 4:00 p.m., Monday through Friday.

Once we receive your request we are usually able to process it within five (5) business days. If you have questions regarding the status of your request, or about completing the form, contact the HIM Department at (208) 883-6092.

Patient fees for medical records


  • Pages 1-30: No Charge
  • Page 31 and over: Flat Fee of $6.50 


  • CD: Flat Fee of $6.50 
  • USB flash drive: Flat Fee of $11.00

Third-Party Requestors

  • Rates upon request

No charge for University of Idaho students with valid University of Idaho identification.

Requesting records on behalf a patient

Medical records can be obtained for a family member if you are the legal guardian or have health care power of attorney for that person. The patient may also request records be released to a personal representative. Please contact the HIM Department for more information on requesting records for a family member.

Correction or amendment of medical records

If you feel there is an error in your health information (medical record), please contact the HIM Department.

Birth certificates

Birth certificates for babies born at Gritman Medical Center are issued by the Idaho Bureau of Vital Records and Health Statistics and mailed 4-6 weeks after the birth. If you have not received your baby's birth certificate within six (6) weeks, please contact:
