Note: Before starting, gather your medical information and be sure that you have sufficient time to fill out this form in one session.

Note: In accordance with Gritman Medical Center policy, our clinics do not prescribe opioids and benzodiazepines concurrently due to safety concerns. Prescriptions for other controlled substances will be considered based on medical necessity and at the provider's discretion.
