FollowMyHealth getting started

1. Provide your email address during registration.
2. After admission, you will receive an email from the Patient Portal.
3. Use the link in the email to launch the online Patient Portal.
4. Click ‘Sign Up and Connect’.
5. Create your username and password.

Connect to your FollowMyHealth account in these 5 easy steps

Step 1 Welcome (click next)

Step 2 Accept the terms of service (click accept)

Step 3 Enter invite code (your 4 digit birth)

Step 4 Release of information (click “I accept”)

Step 5 Congratulations! Your account is ready!
