Our Community Health Needs Assessment

We conduct a Community Health Needs Assessment every three years to evaluate the overall health needs of our region.

The 2023-25 Community Health Needs Assessment (CHNA) identified these top areas of focus:

  • Behavioral Health: Mental Health, Alcohol/Substance Abuse
  • Access and Affordability of Health Care
  • Prevention
  • Chronic Disease Management: Diabetes, Cancer
